Complaint Procedure for the Bureau of Health Systems (BHS)
The Complaint Investigation Unit receives and processes consumer
complaints against nursing homes, home health agencies, hospices,
hospitals, freestanding surgical units, ambulatory surgical units, and
end-stage renal dialysis centers. See A Citizen's Guide to Filing a
Complaint Against a Health Care Facility. The Unit also receives and
processes nursing home reported incidents. Facilities may click here
to link to the incident reporting forms.

THREE METHODS FOR FILING A COMPLAINT
1. Completing the BHS Health Facility Online Complaint Form. THIS FORM
IS ONLY FOR INDIVIDUALS THAT WANT TO FILE A COMPLAINT.
2. Calling the toll-free Complaint Hotline, 1-800-882-6006.
3. Mailing or faxing a completed Nursing Home Complaint Form (BHS-
OPS-361a for nursing homes only), or a Resident/Patient Care Complaint
Form (BHS-OPS-361 for health facilities other than nursing homes),
that can be obtained by clicking on the form name and printing, or by
submitting a letter that includes:

I have been looking over this information regarding filing complaints in Michigan. For specific complaints regarding Substance Abuse Programs there are additional options regarding violations of your rights. Here is a link to take you to Substance Abuse Licensure. Scroll down and review the file "Model Recipient Rights" and then the "Recipient Rights Complaint Form".

Some rights of interest to consider:

"A program staff member shall not physically or mentally abuse or neglect or sexually abuse a recipient..."

"A recipient is entitled to receive an explanatin of his/her bill upon request, regardless of the source of payment"

"A recipeint shall have the benefits, side effects and risks associated with the use of any drugs fully explained in language which is understood by the recipient"

"A recipient has the right to deposit money, earnings, or income in his or her name in an account with a commercial financial institution. A recipient has the right to get money from the account and to spend it or use it as he or she chooses... A recipient has the right to receive all money or other belongings held for him or her by the program within 24 hours of discharge.


Please take some time to look at this information and exercise your rights as provided by the Michigan Legislature. I do not see any fees associated with making a complaint.

I recently spoke to the Victim's Right Advocate in Michigan regarding my ongoing complaint with Narconon Stonehawk. I had filed another complaint. I understand that complaints are still filtering in, please keep up the good work! I have found the Michigan Substance Abuse authorities to be interested, kind and responsive.
